Client Feedback Survey

Collect useful client feedback, identify specific friction, and decide what to improve after a project or meaningful milestone.

What you receive
A 10-page PDF containing client-facing survey questions, rating scales, conditional advocacy questions, an internal interpretation guide, response triage, recommendation-score guidance, form setup notes, and a follow-up workflow. This is a guide for building your survey, not a hosted survey or an Excel dashboard.

Example from the PDF
“What is the one thing we could improve about the experience?” is an optional open question. The guide keeps communication, quality, visibility, timeline, approvals, and value as separate dimensions so a high overall score does not hide a specific problem.

How to use it
1. Choose the post-project, milestone, or retainer-review version.
2. Copy the relevant questions into your preferred form tool, pre-fill known details, and test the client experience on mobile.
3. Review the responses, record recurring themes, and assign an owner and next action when a change is justified.

Who it helps and where it stops
For freelancers, consultants, agencies, and small client-service teams. A small number of responses cannot establish a reliable market benchmark. Positive feedback is not permission to publish a testimonial; obtain the relevant consent separately.
